Hi. Just a bit confused with the photo above that I posted which shows the BK3120 on Ebay that is powered up and has other leds on along with io err led. Makes me think that my bk3120 is faulty.

Did you try to disconnect the power supply for the power bus on + and - and test it again. You will need this supply later of course, but only to see, if yout coupler is doing the same when this supply is disconnected. As I wrote before, I'm not sure if there is another firmware for this coupler where the LEDs are working different.

Yes I tried that but it made no difference.
I have currently 6 faulty BK3120 couplers, that behave all the same as described by the author. After connecting the supply voltage, only the I/O ERR diode is lit (and the green for power). The Master PLC doesn't see the device. When I connect new BK3120 to supply, it blinks all diodes first and the RUN stays on. You don't have to connect anything else, just 2 wires. I can not see any visibly damaged parts inside, when dismantled. So I suppose it is a controller fault, but I don't understand, why we have so many faulty devices recently.
